Vancomycin, Trough

Preferred specimen:	0.5 mL serum, red top tube or SST. Red top tube: Remove serum from cells within 2 hours of collection and transfer to a plastic transport tube. Separator tube (SST): Centrifuge within 2 hours of collection and transport SST refrigerated within 48 hours or transfer serum to a plastic transport tube.
Minimum specimen:	0.2 mL serum or plasma
Notes:	Draw trough immediately prior to next dose.
Acceptable specimen:	0.5 mL plasma, lavender (EDTA) or green (sodium or lithium heparin) top tube. Remove plasma from cells within 2 hours of collection and transfer to a plastic transport tube.
