Test Information
Special preparation is sometimes needed prior to testing as instructed by your health
care provider. Please bring the following items when visiting one of our Patient
Service Centers:
- The test request form provided by your physician
- Your current insurance information
- For drug or paternity testing, proof of identity (drivers license, employee badge, etc)
